Reclaim Form

Please fill in this form to submit a reclaim request for your ball ticket. When you submitted, the Treasurer will receive an email with your information and will process your request. Once processed, you will receive a confirmation email.

By submitting a reclaim form you should also be aware that Van Mildert JCR will temporarily store your information so they can respond to your claim. If you have any GDPR questions or concerns, please email president@mildert.co.uk.
