Reclaim Form

Please fill in this form to submit a reclaim request for your ball ticket. When you have submitted, the Treasurer will receive an email with your information and will process your request within 21 days. Once processed, you will receive a confirmation email.

If you have any questions, or have any technical difficulties when submitting the form, please contact the President.

By submitting a reclaim form you should also be aware that Van Mildert JCR will temporarily store your information so they can respond to your claim. If you have any GDPR questions or concerns, please email president@mildert.co.uk.
